Referrals:
If your primary care provider has sent a referral, please allow up to one week for processing. Due to a high volume of incoming referrals, it is advisable to follow up by calling our office, as referrals are processed in the order received.

New Patients:
We are currently accepting new patients. Please verify with your insurance whether a referral is required. Most HMO plans do require a referral, while PPO plans typically do not. It is the patient’s responsibility to confirm this with their insurance. If a referral is not received by the time of your visit, self-pay rates may apply. Our self-pay rates are $170 for the first visit and $90 for follow-up appointments.

• If your condition worsens or becomes urgent, please seek care at the nearest emergency room and notify your primary care provider as soon as possible.
